At Graceful Transitions Home Veterinary Care, we offer calm, supportive, and comfort-focused veterinary care for pets as they approach the sunset of their lives. Serving families across the North Shore, we provide senior pet consultations, in-home palliative care, quality of life assessments, and gentle euthanasia—available as home visits or virtual consultations.

Our mobile model is designed to reduce stress and increase comfort. Whether it’s an at-home visit or a virtual check-in, our care is guided by empathy, medical insight, and a deep respect for the human-animal bond.

At Graceful Transitions, we do not offer diagnostics, but instead, when they are necessary, we work in collaboration with your primary care vet to ensure continuity of care and the best possible patient outcomes.

How Graceful Transitions Began

Founded by Dr. Peterson, Graceful Transitions grew from a profound desire to help families navigate one of life’s most emotionally complex experiences: a beloved pet’s final chapter. With more than 12 years of experience as an emergency veterinarian, Dr. Peterson witnessed firsthand how difficult and confronting end-of-life decisions can be—and how much guidance, time, and emotional presence families truly need.

Graceful Transitions was created to slow this process down. With longer, more comprehensive consultations, Dr. Peterson is able to take a fully integrated and individualised approach to each patient.

This allows her to work with families to address pain management, mobility challenges, and emotional well-being in a calm, supportive setting. The goal isn’t just to prolong life—it’s to protect quality of life in a way that feels right for both pet and family.
